Display only flashes on first power up

I just purchased a model B+ started kit from amazon.

Unpn first powering up the pi the screen will mostly stay black or occasionally flash what looks like a keyboard layout menu. Both the green and bed lights are constantly lit.

How can I tell if this unit is DOA or can I fix this? The video connection is as follows: Pi -> HDMI cable -> HDMI/DVI converter -> KVM/Monitor.

Start by replacing that SDCard.
Note: Any requirement to use a crystal ball or mind reading will result in me ignoring your question.

I got it to boot during boot up by pressing 2 but now the Pi seems to be stuck in HDMI safemode. This limits my screen res to 640x480. My monitor supports 1080P Nativity, so I am not sure why safemode is necessary. How can I deal with this?
