Journal commit i/o error

I run into these IO errors after about a day. They started happening about a week and a half ago. Before that, no issues at all.
I have a 500MB 2.5" disk containing OS and swap. No overclocking. Latest updates raspbian.
Disk checked multiple times. Fsck gives no errors. ran short test with smartctl and no errors as well...

I am at a bit of a loss what causes this as when the issue occurs, it cannot write to disk anymore (remounted RO) and there are too many errors on screen to retrace back.

Has anyone seen this? Any help greatly appreciated.

by pure luck I saw the following message appear:
hub 1-1:1.0: port 2 disabled by hub (EMI?), re-enabling
That causes the disk to be disconnected hence the journal errors...
Not sure if a hub, a power or a port issue.

I have also received the error in the same manner. However, my Pi is running off an SD card, and has a 1.0Amp power supply. I have not been able to trace what is causing this error to happen.
