Sounds like either your pi or your power supply is fried. Also the potential is there that an accessory or something plugged into your pi is pulling too much power.

First remove all peripherals and try a fresh boot with a new install of Raspbian to rule out any potential external/software issues. Then try replacing the power supply. If the issue persists after all that your pi is the issue and you're going to need a new one.
