Get a screen with a rainbow coloured square on it; it doesn't do anything.

Bought a Pi Zero.
Bought a Transcend branded 4GB Micro SD card
Downloaded and installed Raspberry Pi Imager(Windows 10)
Wrote and verified Raspberry Pi OS Lite (32-bit)
Plugged in HDMI via adapter and keyboard via USB OTG cable.
Plugged in power.
Get a screen with a rainbow coloured square on it; it doesn't do anything.

Can this be fixed or is it just broken and I need to send it back?

When reaching the square does it do anything else?
Does it shutdown, does it repeat booting itself, or just sit there on the screen

Raspbian lite is a gui-less operating system, you probably want the desktop version but that will require at least 5GB of space, so will require an 8GB or larger card.

Usually means the uSD card has issues.
On other Pi's you see the red power led go on and then the green led flickers when it reads the uSD.
A bit of practice and it is easy to spot when the uSD is going to fail.

Sometimes it can just be a dirty contact, I just give them a polish/rub with my thumb.
The thumbprint texture is enough to clean the contacts.

4GB is old card, hope you didn't pay much for it.
I prefer Sandisk 16GB A1 at the moment.

I think it means it has booted from the uSD but has not found a kernel?
I got something like that a few weeks ago, config etc was fine but the kernel was corrupted?

Oh dear, that's about a quarter or eighth of the size you need.
Get a 16GB or 32GB SDCard to replace it.

And, as posted above, don't use the Lite version if you want to display any graphics.
The Lite OS will only give you a console to login to.
