I noticed noone replied to your post, so I thought I'd give it a shot.
As far as downloading a NES emulator on the RPi4, it's pretty easy:

I have already installed it, so yours won't look the same.
As far as making Tetris load just by double clicking an icon:
You can make a desktop launcher. If you create a launcher (right click on the desktop and select "create launcher") you can have it run mednaffe and load a rom in the commandline. Here's an example : mednaffe ~/nesroms/radracer.nes
You can even find an image somewhere on the internet and use it as the icon. I've done this with several Doom mods that I like playing often.
The problem with the setup above, however, is that once you double click the mednaffe icon, it opens mednaffe, and has the rom selected... But it does not launch automatically (you have to click launch). I'm sure there's a way to force mednaffe to run the game at startup, but I haven't figured it out.
Perhaps you can make it work?
There are other emulators out there that work great on the Rpi4. Retroarch, PPSSPP, etc.
