I am using Linux "raspberrypi 4.19.75-v7l+ #1270 SMP Tue Sep 24 18:51:41 BST 2019 armv7l GNU/Linux" (Buster) OS on pi.
Tried 5V 3A DC as well as 5V 4A DC supply to Pi.

Are you using a non standard ribbon cable?

I would undo all the camera connections, make sure everything is clean then reseat . Including the connection from the camera module to the camera pcb.

Thank you very much for reply Jamesh. I got it solved by replacing camera module. Turned out it became faulty. Now all is good with new camera module.
