XBMC may be not setting maximum range.
First, run hello_audio from standard raspbian/wheezy image which does output a full range signal. Is that loud enough? If not, then you'll never get louder.
If that is okay, then xbmc needs tweaking. I assume you've set xbmc volume to maximum? It may be possible to increase that maximum level, but it will result in clipping when audio is full range.
Is it 5.1 audio in the file? Downmixing does reduce volume.
There is a menu option for "boost volume on downmix" which didn't used to do anything. I've made it amplify on the latest xbmc tree, but I'm not sure if openELEC is using that yet.
