Is my RPi dead?

Hi,
My new model B+ has been working fine for over a month, however since today it fails to boot. Both the LEDs (Red-power and Green-ACT) are constantly on as soon as I power it on. The LEDs have same behavior even if I don't insert the microSD. I checked the microSD on another RPi and that seems to be working fine.
What does it mean when both the green and red LEDs are constantly on? Should I consider my RPi dead?

Red light on solid = Power Good.

Green light on solid = Can't read SD card.
Try another card/check the contents/clean the contacts/make sure it is seated correctly.

I was under the impression that both lights solid denotes it is not finding the required information to boot

That hasn't been updated for the B+ where the meaning of the LED status changes.

The wiki not being updated is one reason its being deprecated.

Thanks all for the help/inputs. It indeed was a card problem. I checked with a brand new card and the pi seems to be booting (at least the LEDs blink, will check thoroughly later). Interestingly, the earlier 'bad' card works fine on my model B (it was a fresh NOOBS 1.3.9 card).
