Are you sure you're using a HDMI-to-DVI converter and not a HDMI-to-VGA converter? No active conversion is needed from HDMI to DVI since they're both, similar if not identical, digital video signals and the "conversion" is basically connecting one type of "plug" to another. HDMi to VGA however does require converting a digital signal to a (set of) analogue signals.
Trev.
Still running Raspbian Jessie or Stretch on some older Pi's (an A, B1, 2xB2, B+, P2B, 3xP0, P0W, 2xP3A+, P3B+, P3B, B+, and a A+) but Buster on the P4B's. See: https://www.cpmspectrepi.uk/raspberry_pi/raspiidx.htm

Are both LEDs lit on your RPi, or is your power supply failing? Are you able to test your DVI-D to HDMI cable with some other HDMI device? Have you messed up the front panel settings for your monitor (some let you disable the unused video interfaces)?

Can you try a friend's RPi on your monitor? Can you try your RPi on a TV or a friend's TV?

Both leds are glowing normally, I am using same cable to connect my cpu to the monitor and it is working fine and I have never changed my monitor settings.

This does NOT show that the PI has booted, actually the data LED burning permanently shows it is NOT booting.
Please read the boot problem sticky.

Also yes, no "converter" is needed for DVI-D, you are using an adapter, which simply consists of two connectors wired together. You can replace it with a HDMI to DVI-D cable, and for all purposes the PI simply "sees" a HDMI monitor that happens not to support sound.

Yellow?

The only yellow LED is the network one on the ethernet socket and that will blink if the SMSC9514 LAN/USB chip is powered up and ready.

I think you mean green LED, but that doesn't matter. You said your screen sometimes had some wrong pixels before it failed, which indicates the HDMI signal it received was too weak.
You should increase HDMI drive strength, which is possible by changing the text entry "CONFIG_HDMI_BOOST = number" in config.txt, in the boot partition on the PI, accessible on the card from windows, so you don't need a working display to do it.
