First time boot problem - pi2

Hi,
I've just bought my first raspberry pi 2, but I can't seem to boot it.
I use a brand new micro SD card: Sandisk Ultra 16GB UHS-I
I have read the sticky post regarding boot problems and followed the given instructions:
1) Checked the authenticity of the micro SD card with h2testw_1.4.
2) Formatted the micro SD card using SDFormatter v4.0 with the following options: Full [Overwrite], Size Adjustment On.
3) Downloaded NOOBS 1.4.1 and extracted the files to the card root directory (I have also compared the files to the image posted in the online manual).
4) Plugged in HDMI display and keyboard.
5) Plugged in the power supply (standard 5.25V - 2A).

Results:
Both LED (green and red) are constantly on and no signal is received in the monitor.

From what I've read, when the green light is constantly on this means the raspberry cannot read any data from the SD card.
I've also tried to change the power supply, writing the "2015-05-05-raspbian-wheezy" image to the card with Win32DiskImager,
unplugged HDMI and keyboard usb and also attempted another OS like the OpenELEC...
So far nothing...
I've attached a couple of pictures showing the micro SD card inserted, even though it seems to be sitting properly, maybe I miss something?
Please help! I have no idea what else can I do...

That card doesn't look like it is inserted fully. It should go in until it only protrudes slightly past the edge of the board. It should click into place securely.

Thanks for the reply!
Well, there was a 'click' when I inserted the sd card.
I've tried holding the sd card all the way inside with my finger and then power on, but still no change...

That's frustrating. Ordering, waiting for, and unpacking your new gadget only to have it derp out on you when you try it for the first time. Totally been there.

Anyway, If I were in your shoes, I'd try the shotgun approach. Different SD card, different OS image (raspbian or something), different power adapter/cable, and leave it unplugged from HDMI. Power it on after changing everything at once and see if you have a solid green light. If it's happily blinking, work your way backwards to the problem. If it still doesn't work, you'll know right away the only thing left is likely the pi itself.

Bought another micro SD card from the list of recommended cards, still no go.
Help anyone?

Can you try a different computer? Maybe borrow a friend's or get a USB SD card reader.

Then you should RMA your broken RPi and get a replacement or your money back.
