Yes...i had the same problem... fixed using a different HDMI cable and enabling the config.txt hd option !

I'm having the same issue...
I've changed different settings for hd_mode and all the others options just like you...but nothing..
I'm trying another HDMI cable..i hope it will work..!

i'm having the same mistake !!
But I've never see the raspberry PI boot until now because I can't connect it by DVI...

My green LED blank for the first 3-4 minutes and then it goes down... It's label is "ACT"...

maybe we need to modify the configure.txt file in the SD image?
