I couldn't be bothered messing around any more. The SD card with the same image worked perfectly in another Pi; i.e. not reflashed - took the card card out of the broken Pi, inserted it directly in the working Pi. So not wanting to waste any more of my time, I just returned it and got a replacement....

That sounds broken. Should be replaced under warranty. Did anything happen to the board? Dropped it, voltages on GPIO's etc?

I tried the workaround posted by 6by9 in the GitHub issue, unfortunately it still doesn't work.. 5V 3A PSU. OS is irrelevant as there is no difference to the LED flash pattern whether the SD card is inserted or not. However, it is Raspbian lite, and it is Buster.
