"self powered" means the WD 3TB USB drive has its own power supply?
if you power on the USB HDD drive first before turning ON the RPi4B it should work just fine, and not draw any power from the RPi4B.

Come to think of it, I did encountered an almost similar problem on my very first installation on the RPi4B-4G the clicking the menu icon started the app itself without any problems but the desktop icon created later always ask to be run the script as executable tried tweaking the desktop icon entry...

I've removed 20x20x6 heatsinks attached to the SoC using thermal glue.... several times on the RPi3B+ and RPi3B twist and turn using a radio pliers....... seemed easier to do on the RPi3B+ than on the RPi3B, due to the metallic heat spreader on the RPi3B+ then a few scrapes and alcohol wiping to cle...

Where are your speakers connected? where to you expect the sound to come out?

have you checked the sound setting by right clicking on the speaker icon on the top right of your screen?

pretty elementary....

the underlying OS's sound (HDMI, analog) is operational, and you yourself has proven that.
your DIY built app does not output sound at analog

check your app's code (and dependencies) again and rebuild it if necessary.

One might suspect that you have the ribbon cable plugged reversed on the RPi. your ribbon cable is facing inwards usually its supposed to be facing outwards better shut it down immediately remove the ribbon cable on the RPi and before plugging it again, using a tester check the outermost brown cable...

if you wont believe those warning you about possible mistake..... then go ahead and do what you believe is right but you alone bear the responsibility and consequences if your setup wont work or blows up! one of my RPi setup's main power source comes from a 12v power brick. then reduce the 12v to 5....
