Since you're using WordPress, I'd suggest the WP-Table Reloaded plugin for the actual table.  It's easier to maintain than manually creating an HTML table and it makes the data sortable by column, which is nice.

Hostmonster offers SimpleScripts.  I had never heard of either of those but a quick google makes me think it provides basically the same thing.

I use hostmonster.com and have been pretty happy with them.  Shell access is free but you have to contact support to enable it.  It comes with a free domain registration but you can obviously use one you already own.

You should be able to make a one arm proxy by using two IP addresses for the same NIC. Here is a blog post that should get you going.  I'm hoping to do this as well.
