Interestingly all PSU's I tried caused USB failures on boot (without any USB devices plugged in) . I used the USB interface on TV and USB ports installed on boot. Plugged in kybd and that worked but not the mouse. Used powered USB hub as suggested and all works. Looks like you have to be meticulous ...

USB Error - Cannot reset Port

Hi Has anybody seen 'hub 1-1:1.0: cannot reset port 1 (err = -110)' and 'hub 1-1:1.0: cannot reset port 1 (err = -19)' 'hub 1-1:1.0: cannot disable port 1 (err = -19)' I cannot get any keyboard to work on the USB port and runinng debian6 19-04-12 This is a new PI so i am wondering if its faulty?
