Thank you for the links. An official power supply certainly couldn't hurt and at least would rule power out as a possible cause. I am using an apple 10W USB Power Adapter that came with an ipad.
The input is 100–240V~ 0.45A.
The output is 5.1V 2.1A.

How do you enable verbose mode as suggested and where is the info logged? What I do in this case is start hostapd from the command-line in debug mode. Just add to this thread, I get similar results with the dameware ssh client for windows and raspberry pi3.

I can connect with putty

ssh1 protocal:
connection error: Version mismatch, server version is SSH2

ssh2 protocal:
connection error: Could not negotiate diffie-hellman-group1-sha1

Thanks for your reply. You are correct. I have set the pi to broadcast on the same channel that my home wifi is on. I also set the encryption the same (I think) and the password for both the home wifi and the pi's broadcasted ap the same. Doing all of this seemed to help my initial issue of lost pac...

My problem persists. SSH freezes after a few commands (or seconds) after connecting. Reconnecting with putty allows another few commands. Sometimes I cannot even complete typing one command. More information to share. Looks like the MadWiFi driver is for Atheros chipsets. I have not found a driver for the broadcom BCM43438 that supports simultaneous ap+sta on one device but I'm still looking. Maybe the pi3 supports it out of the box? I have made some progress in that I now know that you can describe this configuration as: Concurrent AP/Station mode operation on the same device.
