Hi! I'm experiencing the exactly same sympthoms: 3 sd cards, a lot of PSUs (including a MOLEX2USB home made to use a computer PSU) and after around 250 ping packets if just stops responding.
I've made many tests with the power supply, and different cables until I got 4.90v inside the raspberry (the psu outputs 5.40v, the cable loses 0.20v aprox.).
Kern.log and messages seem fine, no reboots, this is what I get with the latest 2012-10-28 raspbian image just written to a 2 GB sd,
never logged in, only power cable and network plugged in, and pinging from another computer (DHCP works...):
