EXT4-fs error

I recently added a serial-to-usb connection to the RPI console serial port, and monitor the console via my PC. (I normally run headless, using tightvnc & ssh.

I see this error occasionally on the console:

EXT4-fs error (device mmcblk0p2): ext4_lookup:1043: inode #32: comm java: '.bash_logout' linked to parent dir

mmcblk0p2 is my SD card.

I tried once to copy .bash_logout to a tmp file, deleted the original, then copied the tmp file it back to the original name, thinking to get rid of any "bad" link. However, this issue returned a few days later.

Not sure what causes this, or whether I should worry or not.

Sounds like a problem I have been getting..

I think I have finally nailed it down to a power supply problem (mine drops to 4.69 at time with Network, Powered USB Hub (Which still draws some power) HDMI and card)

Even though I'm running an iPad 3 adapter (and tried iPad 2, HTC Desire and HTC One-X wall chargers) I just can't seem to get it above the magic 4.8 from the test pins.. I'm guessing that the two Micro USB cables I have (both HTC ones) just aren't supplying enough juice as at least two of the chargers I have here are known to work with the RPi :(

Thanks, I'm measuring 4.91 v at the GPIO pins, so not sure this is my problem.

Anyone else seeing this problem or have an idea how to track down the cause ? Thanks!

Still getting this error on the serial console. Here's the last few. They seem to come in groups of three, each of the groups separated by approximately the same amount of time, and within each group, separated by smaller times but similar to the other groups. So it seems periodic.
